Overview

ENA-001 (formerly GAL-021) is a first-in-class small molecule respiratory stimulant developed by Enalare Therapeutics. It acts primarily as an antagonist of the voltage-gated “big” potassium (BK or BKCa) channels located in the carotid body. By inhibiting these channels, ENA-001 stimulates respiration and increases minute ventilation through increased tidal volume and modestly increased ventilatory rate. The drug is administered intravenously and has demonstrated dose-dependent stimulation of breathing in both preclinical models and human studies. ENA-001 is being investigated for use in reversing respiratory depression caused by opioids and non-opioid agents (such as propofol), post-operative respiratory depression, apnea of prematurity, community drug overdose—including xylazine/fentanyl combinations—and COVID‑19-related respiratory disorders. It is considered an "agnostic" respiratory reversal agent because it can counteract various causes of drug-induced hypoventilation without reversing analgesia or precipitating withdrawal[1][6][7][8].
