Enamel matrix derivative (EMD) is a purified extract of porcine fetal tooth material used to biomimetically stimulate the regeneration of soft and hard tissues surrounding teeth following tissue destruction. The biological basis for EMD's effectiveness stems from the discovery that enamel matrix proteins laid down by Hertwig's epithelial root sheath serve as precursors to acellular cementum during its formation (cementogenesis). This acellular cementum signals the development of periodontal ligament fibers, followed by new alveolar bone, leading to the formation of periodontal tissues[9]. EMD is primarily composed of amelogenins (90% of the proteins) and is believed to mimic the development of tooth supporting apparatus during tooth formation[1]. When applied to periodontal lesions, it creates a suitable environment for selective periodontal cell migration and attachment, which helps re-establish lost tooth supporting tissues[8]. The protein self-assembles into an insoluble three-dimensional matrix under physiologic conditions, and is degraded by enzymatic processes of normal wound healing[8].
