Engineered OX40L CAR-Tregs (v2.1) is a preclinical-stage cell therapy candidate developed by Regatta Medical (Regatta Bio) for the treatment of transplant rejection and autoimmune diseases. The therapy utilizes regulatory T cells (Tregs) that have been genetically modified to express a chimeric antigen receptor (CAR) specific for OX40 ligand (OX40L, TNFSF4). This approach is designed to enhance the specificity and potency of the Tregs by targeting activated antigen-presenting cells (APCs) that express OX40L, thereby modulating alloreactive immune responses and promoting immune tolerance. A distinguishing feature of the v2.1 candidate is its engineering for IL-2 independence, which aims to improve the persistence and suppressive function of the CAR-Tregs in vivo without the requirement for exogenous IL-2 supplementation.
