Overview
The Enhanced regimen is a specific preoperative antibiotic prophylaxis protocol developed by Mansoura University for patients undergoing retrograde intrarenal surgery (RIRS) who are at high risk for infectious complications. The regimen consists of a short-course (2-day) administration of antibiotics, primarily utilizing sulfamethoxazole-trimethoprim (TMP-SMX) twice daily, with the final dose administered one day prior to surgery. In instances of patient allergy or bacterial resistance, the protocol allows for the use of nitrofurantoin, ciprofloxacin, or cefpodoxime as alternatives. This regimen is currently being evaluated in a Phase 3 randomized controlled trial against an Extended regimen (7-day course) to determine its efficacy in preventing postoperative urosepsis and other infections while potentially reducing overall antibiotic exposure.
