Enlicitide decanoate + levothyroxine is a combination of two oral pharmaceutical agents: enlicitide decanoate, a novel orally bioavailable macrocyclic peptide inhibitor of proprotein convertase subtilisin/kexin type 9 (PCSK9), and levothyroxine, a synthetic form of the thyroid hormone thyroxine (T4). Enlicitide decanoate is under development as the first oral PCSK9 inhibitor for the reduction of low-density lipoprotein cholesterol (LDL-C), with proven efficacy in phase 3 clinical trials for hyperlipidemia and atherosclerotic cardiovascular disease by blocking PCSK9 and enhancing LDL receptor activity, thereby decreasing plasma LDL-C. Levothyroxine is widely used for replacement therapy in hypothyroidism and suppressive therapy of benign thyroid nodules and thyroid carcinoma, acting as an agonist at nuclear thyroid hormone receptors to regulate metabolic and developmental genes. The combination has been studied for pharmacokinetic interactions and co-administration feasibility in healthy adults, but is not marketed as a fixed-dose combination[1][2][4].
