Enoblituzumab + pembrolizumab is an investigational combination therapy consisting of two monoclonal antibodies targeting distinct immune checkpoint pathways. Enoblituzumab is a humanized, Fc-engineered IgG1 monoclonal antibody that targets B7-H3 (CD276), a member of the B7 family highly expressed in various cancers and associated with poor prognosis. It enhances antibody-dependent cellular cytotoxicity (ADCC) and modulates innate immunity. Pembrolizumab is a humanized IgG4 monoclonal antibody that blocks the programmed cell death 1 receptor (PD-1), thereby releasing inhibition on T cells and promoting adaptive antitumor immunity. The rationale for combining these agents is to achieve coordinated engagement of both innate and adaptive immune responses by simultaneously blocking B7-H3-mediated immunosuppression and PD-1-mediated T-cell exhaustion, potentially resulting in additive or synergistic antitumor activity[2][3][6]. This combination has been evaluated primarily in patients with advanced solid tumors, including non-small cell lung cancer (NSCLC) and head and neck squamous cell carcinoma (HNSCC), particularly those naïve to prior checkpoint inhibitor therapy[1][2][5].
