This is a combination of three oral antiviral agents—entecavir, tenofovir disoproxil fumarate, and adefovir dipivoxil—all of which are nucleos(t)ide analogue reverse transcriptase inhibitors used in the treatment of chronic hepatitis B virus (HBV) infection. Each drug inhibits HBV DNA polymerase by competing with natural substrates and causing chain termination during viral replication. Entecavir is known for its high potency and low resistance profile; tenofovir disoproxil fumarate is highly effective against both wild-type and resistant HBV strains; adefovir dipivoxil has activity against lamivudine-resistant HBV but lower potency compared to the other two agents. This triple combination would be considered in cases of multidrug-resistant chronic hepatitis B where monotherapy or dual therapy has failed, although such combinations are not standard first-line regimens. The use of all three together aims to maximize viral suppression in patients with complex resistance patterns.
