This combination therapy consists of the androgen receptor antagonist enzalutamide, the antimetabolite gemcitabine, and the albumin-bound microtubule inhibitor nab-paclitaxel. It was investigated by the H. Lee Moffitt Cancer Center and Research Institute in a Phase 1 clinical trial (NCT02138383) for the treatment of metastatic or unresectable pancreatic adenocarcinoma. The rationale for adding enzalutamide to the standard-of-care gemcitabine and nab-paclitaxel regimen is based on evidence that androgen receptor signaling may play a role in the progression of pancreatic cancer. Enzalutamide works by competitively inhibiting androgen binding to the androgen receptor, while gemcitabine inhibits DNA synthesis as a nucleoside analog, and nab-paclitaxel stabilizes microtubules to prevent cell division.
