Epalrestat is a small molecule, carboxylic acid derivative that acts as a noncompetitive and reversible inhibitor of aldose reductase, the key enzyme in the polyol pathway. By inhibiting aldose reductase, epalrestat reduces the accumulation of intracellular sorbitol, which is implicated in the pathogenesis of diabetic complications such as neuropathy, retinopathy, and nephropathy. Epalrestat is primarily used for the treatment of diabetic peripheral neuropathy and has demonstrated efficacy in improving nerve conduction velocity and subjective symptoms associated with this condition. It is well tolerated with gastrointestinal disturbances being among the most common adverse effects. Epalrestat is approved for use in several Asian countries including Japan, India, China, and Bangladesh[1][3][6][7].
