EPB-53 is a small molecule activator of the heme-regulated inhibitor (HRI), also known as eukaryotic translation initiation factor 2-alpha kinase 1 (EIF2AK1). By activating HRI, EPB-53 triggers the phosphorylation of eIF2α, which subsequently induces the expression of fibroblast growth factor 21 (FGF21), a hormone critical for lipid homeostasis and metabolic regulation. Developed through a collaboration between the University of Barcelona and the University of Louisville, EPB-53 has demonstrated the ability to alleviate hepatic steatosis and prevent the transition from non-alcoholic steatohepatitis (NASH) to hepatocellular carcinoma (HCC) in murine models. Its mechanism involves lipid metabolic reprogramming, downregulation of the SPHK1-S1P-HIPPO-YAP signaling pathway, and modulation of the tumor immune microenvironment, particularly by restoring the function of macrophages and Kupffer cells.
