Overview

**Epcoritamab + ibrutinib** is an investigational combination therapy being evaluated for the treatment of relapsed or refractory aggressive B-cell non-Hodgkin lymphoma and related B-cell lymphomas. Epcoritamab is a bispecific monoclonal antibody that binds to CD3 on T cells and CD20 on B cells, functioning as a T-cell engager to mediate T-cell–dependent cytotoxicity against malignant B cells. Ibrutinib is a small molecule Bruton's tyrosine kinase (BTK) inhibitor; it disrupts B-cell receptor signaling, leading to impaired B-cell survival and proliferation, and additionally modulates immune responses through inhibition of interleukin-2-inducible T-cell kinase (ITK). The combination is hypothesized to provide complementary antineoplastic effects: epcoritamab directly activates T cells to target lymphoma cells, while ibrutinib reduces B-cell survival and may improve T-cell fitness, synergistically enhancing the cytotoxic effect against B-cell malignancies[1][3][5][7][9].
