Epcoritamab + lenalidomide is an investigational combination therapy for relapsed or refractory diffuse large B-cell lymphoma (R/R DLBCL) and follicular lymphoma. **Epcoritamab** is a subcutaneous bispecific monoclonal antibody that simultaneously binds CD3 on T cells and CD20 on B cells, facilitating T-cell mediated cytotoxicity against malignant B cells. **Lenalidomide** is an orally administered immunomodulatory small molecule that enhances immune cell function and inhibits tumor angiogenesis and proliferation. Combined, these agents provide a chemotherapy-free regimen designed to enhance T-cell proliferation and augment natural killer cell activity, potentially leading to increased antitumor cytotoxicity. In clinical trials (EPCORE NHL-5, NCT05283720; EPCORE FL-1, NCT05409066), the combination has demonstrated high response rates, deep and durable remissions, and a manageable safety profile. The combination is being developed primarily by Genmab and AbbVie as treatment for B-cell lymphomas[1][3][5].
