Epcoritamab + loncastuximab tesirine is a combination therapy being investigated for the treatment of relapsed or refractory large B-cell lymphoma (LBCL). Epcoritamab is a bispecific T-cell engager (BiTE) that simultaneously binds to CD3 on T cells and CD20 on B cells, facilitating T-cell-mediated lysis of malignant B cells. Loncastuximab tesirine is a CD19-directed antibody-drug conjugate (ADC) consisting of a humanized monoclonal antibody conjugated to a pyrrolobenzodiazepine (PBD) dimer cytotoxic alkylating agent. The combination aims to enhance anti-tumor activity by targeting multiple B-cell antigens (CD19 and CD20) and employing distinct mechanisms of action, including T-cell recruitment and direct DNA damage. This regimen is currently being evaluated in a Phase 2 clinical trial led by the University of Miami.
