Epeleuton is a novel, orally administered, synthetic second-generation omega-3 fatty acid drug, specifically the ethyl ester of 15-hydroxy eicosapentaenoic acid (15(S)-HEPE). It acts as a small molecule with anti-inflammatory and pro-resolving properties. Epeleuton is being developed primarily for the treatment of sickle cell disease (SCD), where it targets both red blood cells and vascular inflammation—addressing hemolysis, anemia, endothelial adhesion, organ damage, and inflammatory vasculopathy. Its mechanism involves modulation of lipidomic patterns in target organs toward a pro-resolving state and inhibition of key inflammatory pathways. Epeleuton has also shown potential in other indications such as hypertriglyceridemia, non-alcoholic fatty liver disease (NAFLD), non-alcoholic steatohepatitis (NASH), type 2 diabetes mellitus, idiopathic pulmonary fibrosis (IPF), diabetic neuropathies, alcoholic hepatitis, chronic obstructive pulmonary disease (COPD), and others[1][2][5][6][7].
