ePGT121v1-LS is an engineered, broadly neutralizing monoclonal antibody (bNAb) designed for the prevention and treatment of HIV-1 infection. Developed by the National Institute of Allergy and Infectious Diseases (NIAID), it is an optimized version of the parental PGT121 antibody, targeting the V3-glycan supersite on the HIV-1 envelope glycoprotein (gp120). The 'LS' suffix indicates a two-amino acid substitution (M428L/N434S) in the Fc region, which enhances binding affinity for the neonatal Fc receptor (FcRn). This modification increases the antibody's serum half-life through enhanced endosomal recycling, potentially allowing for less frequent dosing. In clinical development, ePGT121v1-LS is being evaluated both as a monotherapy and in combination with other bNAbs, such as PGDM1400LS and VRC07-523LS, to provide broader coverage against diverse HIV-1 strains and reduce the risk of viral escape.
