Overview

EPI-7386 + enzalutamide is an investigational combination therapy for metastatic castration-resistant prostate cancer (mCRPC). EPI-7386 is a next-generation "aniten," a small molecule inhibitor that targets the N-terminal domain of the androgen receptor (AR), blocking AR activity even in the presence of resistance mechanisms affecting the ligand-binding domain, such as point mutations and splice variants. Enzalutamide is an approved second-generation antiandrogen that inhibits AR signaling by binding to its ligand-binding domain. The combination aims to achieve a deeper blockade of the AR pathway than either agent alone, potentially overcoming resistance and improving antitumor activity. Clinical trials have shown rapid and significant PSA reductions with this regimen, and it is currently being evaluated in Phase 1/2 studies for safety, pharmacokinetics, and efficacy in mCRPC patients[1][2][3][5].
