Overview

This is a combination chemotherapy and immunotherapy regimen consisting of five agents: - **Epirubicin** (an anthracycline antibiotic, antineoplastic agent) - **Cyclophosphamide** (an alkylating agent) - **Vincristine** (a vinca alkaloid, antineoplastic agent) - **Prednisone** (a synthetic glucocorticoid corticosteroid) - **Rituximab** (a chimeric monoclonal antibody targeting CD20 on B cells) The regimen is used primarily for the treatment of aggressive B-cell lymphomas such as diffuse large B-cell lymphoma (DLBCL), especially in elderly or frail patients where modifications to standard regimens like R-CHOP may be needed. Each component has a distinct mechanism: - Epirubicin intercalates DNA and inhibits topoisomerase II. - Cyclophosphamide crosslinks DNA, leading to cell death. - Vincristine disrupts microtubule formation, inhibiting mitosis. - Prednisone induces apoptosis in certain cancerous cells via glucocorticoid receptor activation. - Rituximab binds CD20 on B lymphocytes, mediating cell lysis through immune mechanisms. This combination leverages both cytotoxic and immunotherapeutic effects for synergistic anti-tumor activity[2][4][6].
