**EpiSmart** is an investigational epithelium-on corneal cross-linking drug-device combination developed by Epion Therapeutics for keratoconus and ectatic corneal disease. The system integrates EpiPrep corneal drug-delivery applicators, RiboStat ophthalmic riboflavin and sodium iodide formulation, and a UVA light device. Topically delivered riboflavin is photoactivated by UVA light to generate singlet oxygen and induce corneal stromal collagen cross-links, thereby strengthening the biomechanically weakened cornea while preserving the corneal epithelium. EpiSmart completed patient follow-up in its pivotal Phase 3 keratoconus program in June 2026 and remains investigational. ([epiontx.com](https://www.epiontx.com/epismart))
