Overview

This is a combination treatment involving epoetin alfa and packed red blood cell (RBC) transfusion used to manage anemia, particularly in patients with chronic kidney disease, chemotherapy-induced anemia, or other causes of low red blood cell counts. - **Epoetin alfa** is a recombinant human erythropoietin that stimulates erythropoiesis by binding to the erythropoietin receptor on erythroid progenitor cells in the bone marrow. This activates intracellular signaling pathways including JAK2/STAT5 leading to increased red blood cell production. - It is primarily indicated for anemia associated with chronic kidney disease, chemotherapy, HIV therapy (zidovudine), and to reduce the need for RBC transfusions during certain surgeries. - Packed RBC transfusion involves transferring donor red blood cells directly into a patient’s bloodstream to rapidly increase oxygen-carrying capacity in cases of severe anemia. The combination is used when epoetin alfa alone may not be sufficient or rapid correction of severe anemia is required. Epoetin alfa aims to stimulate endogenous production of RBCs over weeks while packed RBC transfusions provide immediate correction.
