Overview

Epoprostenol + phenylephrine is a combination of two pharmacologically active agents with opposing vascular effects, sometimes used together in specific clinical scenarios such as during anesthesia or critical care. Epoprostenol is a synthetic analog of prostacyclin, acting as a potent vasodilator and inhibitor of platelet aggregation, primarily indicated for the treatment of pulmonary arterial hypertension. It works by relaxing vascular smooth muscle via activation of prostacyclin receptors and increasing cyclic AMP levels, leading to vasodilation and reduced pulmonary vascular resistance[1][4][6]. Phenylephrine is an alpha-1 adrenergic receptor agonist that induces vasoconstriction and increases systemic blood pressure by stimulating alpha-1 receptors on vascular smooth muscle[3]. The combination may be used in settings where selective pulmonary vasodilation is desired without causing systemic hypotension; for example, inhaled epoprostenol can improve oxygenation while intravenous phenylephrine maintains systemic blood pressure during one-lung ventilation or other perioperative situations[2]. This combination does not represent a fixed-dose commercial product but rather concurrent administration based on clinical need.
