**Equine antithymocyte globulin** is a horse-derived polyclonal immunoglobulin G preparation used for immunosuppression in aplastic anemia. It is produced from serum of horses immunized with human thymus lymphocytes and reduces pathogenic T-lymphocyte activity that contributes to immune-mediated destruction of hematopoietic stem and progenitor cells. In the NHLBI Phase 2 severe aplastic anemia study, horse antithymocyte globulin was administered intravenously with cyclosporine and mycophenolate mofetil; adding mycophenolate mofetil did not improve outcomes compared with historical horse antithymocyte globulin plus cyclosporine therapy. Equine antithymocyte globulin is marketed as Atgam and is approved in the United States for moderate to severe aplastic anemia in patients unsuitable for bone marrow transplantation.
