Overview

ERAP inhibitor (specifically targeting ERAP1) is a novel therapeutic approach being developed by Grey Wolf Therapeutics for the treatment of axial spondyloarthritis (axSpA). ERAP1 (Endoplasmic Reticulum AminoPeptidase 1) is an enzyme responsible for trimming peptides to the optimal length for presentation by MHC class I molecules, such as HLA-B27. In patients with axial spondyloarthritis, genetic variants of ERAP1 are strongly associated with disease susceptibility, particularly in HLA-B27 positive individuals. By inhibiting ERAP1, the drug aims to alter the repertoire of peptides presented on the cell surface, thereby preventing the presentation of arthritogenic peptides that trigger an inappropriate immune response and chronic inflammation in the spine and joints. While Grey Wolf's lead candidate GRW4352 is currently in clinical trials for oncology, the ERAP inhibitor program for axial spondyloarthritis is in the preclinical development stage.
