Overview

ERC-Exos are exosomes derived from endometrial regenerative cells (ERCs), which are mesenchymal-like stem cells typically isolated from menstrual blood. These exosomes serve as an acellular therapeutic platform being investigated for the treatment of inflammatory bowel diseases, specifically ulcerative colitis. The therapeutic mechanism of ERC-Exos is primarily mediated by the delivery of Sirtuin 6 (SIRT6), a histone deacetylase that modulates the immune environment. By delivering SIRT6, ERC-Exos inhibit the NF-κB signaling pathway in macrophages, which shifts their polarization from a pro-inflammatory M1 phenotype to an anti-inflammatory M2 phenotype. This modulation leads to a decrease in pro-inflammatory cytokines such as IL-6 and TNF-α, and an increase in anti-inflammatory factors like IL-10 and TGF-β, thereby alleviating colonic inflammation and pathological damage.
