Overview

Ergot is a fungal parasite (Claviceps purpurea) that produces various alkaloids with significant pharmacological properties. Historically, ergot has been used medicinally for centuries, though it's also known for causing severe poisoning called ergotism or "St. Anthony's fire" when consumed in contaminated grains. ## Medical Uses and Applications Ergot has been used medicinally for several conditions, particularly in obstetrics and gynecology: - Treatment of excessive bleeding during menstrual periods - Expelling placenta after childbirth - Controlling uterine bleeding and irregular spasms - Treatment of fibrous tumors of the uterus - Prevention of miscarriage in cases of profuse hemorrhage Modern medical applications of ergot derivatives include: - Treatment of migraine headaches (ergotamine) - Increasing uterine contractions during childbirth (ergometrine/ergonovine) - Suppressing prolactin secretion - Inducing vasoconstriction ## Pharmacology Ergot contains high concentrations of alkaloids (up to 2% of dry mass), particularly ergotamine. These alkaloids are classified as: 1. Derivatives of 6,8-dimethylergoline 2. Lysergic acid derivatives The ergot alkaloids work through several mechanisms: - Narrowing blood vessels in the brain - Affecting serotonin levels - Blocking pain signals - Inhibiting the trigeminal nerve from releasing inflammatory proteins - Acting as agonists at various receptors including serotonin, adrenergic, and dopamine receptors ## Safety Concerns Ergot is considered UNSAFE when taken by mouth due to high risk of poisoning that can be fatal. Early symptoms of ergot poisoning include: - Nausea and vomiting - Muscle pain and weakness - Numbness and itching - Irregular heartbeat If untreated, ergot poisoning can progress to: - Gangrene - Vision problems - Confusion - Spasms and convulsions - Unconsciousness - Death Ergot is particularly dangerous during pregnancy and breastfeeding and can worsen heart disease. ## Historical Significance Ergot has a fascinating historical significance. During the Middle Ages, ergotism was common and known as "St. Anthony's fire." Some historians believe ergot played a role in the Salem witch hunt of 1692, suggesting that peculiar behaviors exhibited by some women may have resulted from consuming ergot-contaminated food. ## Modern Pharmaceutical Derivatives While raw ergot is dangerous, certain chemicals derived from ergot are used in prescription medicines. These include: - Ergotamine (for migraines) - Ergometrine/ergonovine (for uterine contractions and controlling bleeding after childbirth) Ergot alkaloids are among the oldest headache treatments, developed in the 1920s and prescribed since the 1940s. They differ from triptans (a newer class of migraine medicine) in that they work more slowly but provide longer-lasting pain relief.
