Ermiao Pill (Er Miao Wan) is a classic Traditional Chinese Medicine (TCM) formula composed of two primary herbal ingredients: *Phellodendron amurense* bark (Huang Bai) and *Atractylodes lancea* rhizome (Cang Zhu). Traditionally used to clear heat and dry dampness, it is indicated for conditions such as gouty arthritis, hyperuricemia, and inflammatory skin diseases. In modern clinical research, particularly studies associated with the Drum Tower Hospital Affiliated to Nanjing University School of Medicine, Ermiao Pill has been investigated for its efficacy in treating herpes zoster neuralgia (postherpetic neuralgia). The formula exerts its therapeutic effects through anti-inflammatory and analgesic mechanisms, largely attributed to active phytochemicals like berberine. These compounds inhibit the production of pro-inflammatory cytokines such as tumor necrosis factor (TNF) and interleukin-6 (IL-6) and modulate pain signaling pathways to alleviate neuropathic pain.
