Ermiao powder is a traditional Chinese herbal formula composed of two main ingredients: Phellodendron amurense bark (Huang Bo) and Atractylodes lancea rhizome (Cang Zhu)[2][3][4]. It was first described in 1481 AD in the "Essential Teachings of Zhu Dan Xi." Ermiao powder is primarily used to expel dampness and drain heat from the lower body ("Lower Burner") according to Traditional Chinese Medicine (TCM) theory[3]. Modern pharmacological studies indicate that Ermiao powder exerts anti-inflammatory effects by modulating immune pathways. Specifically, it has been shown to regulate the cholinergic anti-inflammatory pathway via downregulation of CHRNA7 (encoding α7 nicotinic acetylcholine receptor), as well as targeting key inflammatory signaling pathways such as PI3K-Akt, MAPK, TNF, and NF-κB[1][2]. The formula contains multiple active components including wogonin, paeonol, ethyl caffeate, and magnoflorine[2]. Its primary clinical indication is for conditions associated with damp-heat patterns such as rheumatoid arthritis; it may also be used for other inflammatory or pain-related disorders in TCM practice[1][2][3].
