Erxian Decoction (EXD) is a classic Traditional Chinese Medicine (TCM) formula composed of six herbs: *Herba Epimedii* (Yinyanghuo), *Rhizoma Curculiginis* (Xianmao), *Radix Angelicae Sinensis* (Danggui), *Rhizoma Anemarrhenae* (Zhimu), *Cortex Phellodendri* (Huangbai), and *Radix Morindae Officinalis* (Bajitian). Originally formulated to treat menopausal syndrome by tonifying kidney yang and essence while purging kidney fire, it is now widely investigated for its regulatory effects on the hypothalamic-pituitary-gonadal (HPG) axis. In clinical research, EXD is being evaluated for its efficacy in treating late-onset hypogonadism (LOH) and associated lower urinary tract symptoms (LUTS) in males. Its mechanism of action involves modulating hormone levels, including testosterone and estradiol, and exerting anti-inflammatory and antioxidant effects to improve endocrine and urological function.
