Erythromycin + budesonide is a combination of two drugs with distinct mechanisms of action. Erythromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing bacterial growth. Budesonide is a synthetic corticosteroid that exerts potent anti-inflammatory effects by suppressing multiple inflammatory genes and cytokines through glucocorticoid receptor activation. This combination has been studied for its potential to reduce inflammation and modify fibrosis progression, particularly in the context of tracheal injury or tracheal stenosis (TS). Preclinical studies suggest that the combination may be more effective than either agent alone in reducing markers of inflammation and fibrosis after airway injury[2][9]. However, co-administration can increase systemic absorption of budesonide due to inhibition of its metabolism by erythromycin, potentially increasing side effects[1][3].
