Overview

Erythromycin + metronidazole is a combination of two antibiotics, each with distinct mechanisms of action. Erythromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing translocation and subsequent peptide chain elongation. It is primarily active against Gram-positive bacteria and some Gram-negative organisms. Metronidazole is a nitroimidazole antibiotic and antiprotozoal agent that exerts its effect by entering anaerobic cells and being reduced to reactive intermediates which disrupt DNA structure, leading to cell death. This combination has been studied for use in preventing spontaneous preterm delivery in certain obstetric populations but did not show efficacy for this indication[3]. Both drugs are approved individually for various bacterial infections; however, their combined use should be approached with caution due to potential drug interactions such as increased risk of serious cardiac arrhythmias[1].
