This is a combination of five antidepressant medications—escitalopram, paroxetine, bupropion, venlafaxine, and sertraline. All are oral small molecule pharmaceuticals used primarily in the treatment of major depressive disorder and other mood or anxiety disorders. Escitalopram, paroxetine, and sertraline are selective serotonin reuptake inhibitors (SSRIs), which increase synaptic serotonin by inhibiting its reuptake into presynaptic neurons. Venlafaxine is a serotonin-norepinephrine reuptake inhibitor (SNRI), affecting both serotonin and norepinephrine transporters. Bupropion is a norepinephrine-dopamine reuptake inhibitor (NDRI) that increases synaptic concentrations of norepinephrine and dopamine by inhibiting their respective transporters. Combining multiple antidepressants from different classes may be considered in cases of treatment-resistant depression but carries significant risks for drug interactions—including increased risk for serotonergic toxicity (serotonin syndrome), pharmacokinetic interactions via cytochrome P450 inhibition/induction, additive side effects such as sexual dysfunction or hypertension, and therapeutic duplication[1][2][3][7]. There is limited evidence supporting the efficacy or safety of combining this many antidepressants simultaneously; most clinical studies focus on dual combinations rather than five-drug regimens.
