Overview

Escitalopram + pindolol is a combination of two pharmaceutical agents used experimentally to enhance and accelerate antidepressant response in major depressive disorder. Escitalopram is a selective serotonin re-uptake inhibitor (SSRI) that increases serotonergic neurotransmission by inhibiting the serotonin transporter (SERT), both at the orthosteric and allosteric sites, leading to increased synaptic serotonin levels[1]. Pindolol is a non-selective beta-adrenoceptor antagonist (beta blocker) with partial agonist activity at beta-1 and beta-2 adrenergic receptors as well as antagonistic activity at 5-hydroxytryptamine receptor 1A (5-HT1A)[2][6]. When combined with SSRIs like escitalopram, pindolol may block presynaptic 5-HT1A autoreceptors in the raphe nuclei. This action prevents SSRI-induced inhibition of serotonergic neuron firing early in treatment and can accelerate or potentiate antidepressant effects[6][8]. The combination has been studied for its potential to produce faster onset of antidepressant action compared to SSRI monotherapy.
