Esketamine + ropivacaine is a combination of two drugs used primarily for regional anesthesia and analgesia, especially in obstetric settings such as epidural labor analgesia and postoperative pain management. Esketamine is the S-enantiomer of ketamine, acting as a non-competitive antagonist of the N-methyl-D-aspartate (NMDA) receptor, which provides potent analgesic and anesthetic effects with fewer psychomimetic side effects compared to racemic ketamine. Ropivacaine is a long-acting amide-type local anesthetic that blocks voltage-gated sodium channels in nerve fibers, resulting in sensory and motor block. When combined, esketamine enhances the analgesic effect of ropivacaine, reduces the required dose of local anesthetic for effective pain control, prolongs duration of nerve block, decreases opioid consumption postoperatively, and may reduce incidence of postpartum depression due to its rapid antidepressant properties. This combination has been studied for use via epidural administration during labor and caesarean section as well as peripheral nerve blocks such as transversus abdominis plane block (TAPB). Clinical studies indicate improved patient satisfaction without significant increase in adverse events[1][3][4][5][6].
