Esomeprazole + bismuth tripotassium dicitrate + tetracycline + metronidazole is a bismuth-based quadruple therapy (BQT) regimen being investigated as a frontline treatment for early-stage Helicobacter pylori-negative gastric mucosa-associated lymphoid tissue (MALT) lymphoma. Developed and studied by the National Taiwan University Hospital, this combination therapy aims to eradicate potential occult H. pylori or other bacterial pathogens that may drive the lymphoproliferative process in the gastric mucosa. The regimen includes esomeprazole, a proton pump inhibitor that reduces gastric acidity to stabilize antibiotics; bismuth tripotassium dicitrate, which provides cytoprotection and direct antimicrobial effects; and the antibiotics tetracycline and metronidazole, which provide broad-spectrum coverage against anaerobic and gram-negative bacteria. This approach is based on the observation that a subset of H. pylori-negative MALT lymphoma patients responds to antibiotic therapy, suggesting a bacterial etiology even in the absence of standard diagnostic markers for H. pylori.
