ESP41091 is a small molecule drug candidate that acts as an oral inducer of Apolipoprotein A-I (ApoA-I) transcription. Originally developed by Pfizer, the rights to ESP41091 were acquired by Esperion Therapeutics in April 2008 alongside bempedoic acid (ETC-1002). The therapeutic rationale for ESP41091 was to increase the endogenous production of ApoA-I, the major protein component of high-density lipoprotein (HDL) particles, to enhance reverse cholesterol transport and provide a novel treatment for dyslipidemia and cardiovascular disease. Despite its acquisition, Esperion focused its resources on the development of bempedoic acid, and ESP41091 has remained in early-stage development without progressing into late-phase clinical trials.
