Overview

Estrogen is a steroid hormone that plays a central role in the regulation of the female reproductive system and is known to have significant effects on the cardiovascular system. In the clinical trial Estrogen, Cytokines and Heart Failure in Women (NCT00000608), sponsored by the National Heart, Lung, and Blood Institute (NHLBI), estrogen was evaluated as a potential treatment for postmenopausal women with idiopathic dilated cardiomyopathy (IDCM). The study aimed to determine if estrogen replacement therapy could improve heart failure symptoms, promote reverse ventricular remodeling, and decrease the levels of pro-inflammatory cytokines, which are associated with the progression of heart failure. Estrogen's mechanism of action involves binding to estrogen receptors alpha (ERα) and beta (ERβ), which then modulate the transcription of genes involved in vascular tone, inflammation, and myocardial cell survival.
