Etamiphylline (also known as etamiphyllin) is a xanthine derivative and small molecule drug developed as a respiratory stimulant and bronchodilator, primarily intended for use in asthma and chronic obstructive pulmonary disease (COPD)[1][3][4][5]. It acts as a weaker bronchodilator compared to theophylline when administered orally[4]. Etamiphylline belongs to the class of methylxanthines, which are thought to exert their effects by inhibiting phosphodiesterase enzymes, leading to increased intracellular cyclic AMP levels, and by antagonizing adenosine receptors—mechanisms similar to other xanthines such as aminophylline[6][8]. However, clinical trials have shown poor or absent efficacy in humans for its primary indications[1]. The drug reached up to phase II clinical trials but has not been widely adopted or approved for these uses[3].
