Etanercept + infliximab is a combination of two biologic drugs, both of which are tumor necrosis factor (TNF) inhibitors used to treat autoimmune and inflammatory diseases. Etanercept is a dimeric fusion protein consisting of the extracellular ligand-binding portion of the human p75 TNF receptor linked to the Fc portion of human IgG1, acting as a decoy receptor for TNF-α. Infliximab is a chimeric monoclonal antibody that binds with high affinity to both soluble and transmembrane forms of TNF-α, neutralizing its activity and inducing lysis in cells expressing membrane-bound TNF-α. Both agents reduce inflammation by blocking the pro-inflammatory effects mediated by TNF-α but differ in their molecular structure and some aspects of their mechanism—etanercept cannot lyse cells or bind receptor-bound TNF-α, while infliximab can[1][2][9]. This combination would theoretically provide dual blockade at different points in the inflammatory cascade; however, there is no clinical evidence supporting combined use due to overlapping mechanisms and increased risk for immunosuppression.
