Ethiodized oil (Lipiodol) + indocyanine green (ICG) is a combination agent used primarily for the treatment and intraoperative visualization of hepatocellular carcinoma (HCC). Lipiodol, an ethiodized poppyseed oil, serves as a radio-opaque contrast agent and a carrier for therapeutic agents, preferentially accumulating in the microvasculature of hypervascular HCC tumors due to their lack of Kupffer cells and abnormal lymphatic drainage. Indocyanine green is a near-infrared fluorescent dye that binds to plasma proteins like albumin. When emulsified with Lipiodol and administered via transarterial chemoembolization (TACE) or transarterial embolization (TAE), the mixture allows for both radiographic monitoring of the embolization process and real-time fluorescence-guided surgery. This combination enhances the precision of tumor localization, helping surgeons identify small or deep-seated lesions and achieve complete R0 resection margins.
