Ethylmethylhydroxypyridine succinate is a synthetic small molecule most commonly used in Russia and some CIS countries under the brand name Mexidol. It is the succinate salt form of emoxypine (2-ethyl-6-methyl-3-hydroxypyridine), with the succinate moiety enhancing solubility and biological effects. Its mechanism of action is multimodal, including pronounced antioxidant, antihypoxic, membrane-stabilizing, and neuroprotective effects. The drug acts by neutralizing free radicals, protecting cellular membranes, modulating membrane-bound enzymes and receptors, promoting aerobic glycolysis, and increasing ATP and phosphocreatine content in cells, particularly under hypoxic conditions. The succinate component, as a Krebs cycle intermediate, may further exert effects by stimulating succinate receptors (GPR91/SUCNR1), supporting mitochondrial energy metabolism, and inducing adaptive responses to hypoxia. Developed in Russia, it is widely used as a neuroprotective and nootropic agent, though it is not approved by Western regulatory agencies.[1][3][5][6]
