Ethylmorphine is a semi-synthetic opioid analgesic and antitussive agent derived from morphine. It is used primarily to treat coughs associated with colds and lung infections, as well as for its pain-relieving properties. Ethylmorphine acts mainly as an agonist at the mu-opioid receptor (OPRM1), producing analgesia and suppression of the cough reflex. It is metabolized in the liver by cytochrome P450 enzymes—primarily CYP2D6 (O-deethylation to morphine) and CYP3A4 (N-demethylation)—and its effects are mediated through both direct action on opioid receptors and conversion to active metabolites such as morphine[2][4][6]. Ethylmorphine is not marketed in the United States but remains approved in several other countries; it is classified as a Schedule II controlled substance when used alone due to its high potential for abuse[1][2].
