Overview

Deriphyllin is a fixed-dose combination bronchodilator containing the xanthine derivatives etofylline and theophylline. Developed and marketed by Zydus Healthcare, it is widely used in India for the management of asthma, chronic obstructive pulmonary disease (COPD), and other chronic respiratory conditions. The drug works primarily by non-selectively inhibiting phosphodiesterase (PDE) enzymes, which leads to increased intracellular levels of cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP). This biochemical change results in the relaxation of bronchial smooth muscle and improved airflow to the lungs. Additionally, theophylline acts as an adenosine receptor antagonist and may enhance histone deacetylase 2 (HDAC2) activity, which can help reverse corticosteroid resistance in inflammatory lung diseases. Deriphyllin is available in various formulations, including immediate-release and sustained-release (Retard) tablets, syrups, and ampoules for injection.
