etoposide + cyclophosphamide is a combination chemotherapy regimen being evaluated by Rutgers, The State University of New Jersey, for the treatment of stage D0 prostate cancer. This specific approach utilizes metronomic dosing, which involves the frequent, low-dose administration of oral etoposide and oral cyclophosphamide. Cyclophosphamide acts as a nitrogen mustard alkylating agent that forms DNA cross-links, while etoposide is a semi-synthetic derivative of podophyllotoxin that inhibits DNA topoisomerase II. The metronomic schedule is intended to shift the mechanism from purely cytotoxic to include anti-angiogenic and immunomodulatory effects, potentially slowing disease progression in patients with rising PSA levels after local therapy but before the development of metastatic disease.
