Overview

This specific approach utilizes metronomic dosing, which involves the frequent, low-dose administration of oral etoposide and oral cyclophosphamide. Cyclophosphamide acts as a nitrogen mustard alkylating agent that forms DNA cross-links, while etoposide is a semi-synthetic derivative of podophyllotoxin that inhibits DNA topoisomerase II. The metronomic schedule is intended to shift the mechanism from purely cytotoxic to include anti-angiogenic and immunomodulatory effects, potentially slowing disease progression in patients with rising PSA levels after local therapy but before the development of metastatic disease.
