The combination of etoposide, cytarabine, and pegfilgrastim—often referred to as the EAP regimen—is a chemotherapy-based mobilization protocol primarily used in patients with multiple myeloma or lymphoma who are considered poor mobilizers for autologous stem cell transplantation. - **Etoposide** is a topoisomerase II inhibitor that induces DNA strand breaks, leading to apoptosis in rapidly dividing cells. - **Cytarabine** (also known as Ara-C) is an antimetabolite that inhibits DNA synthesis by acting as a cytidine analog, interfering with DNA polymerase during the S-phase of the cell cycle. - **Pegfilgrastim** is a long-acting recombinant human granulocyte colony-stimulating factor (G-CSF) analog. It stimulates bone marrow to increase production and maturation of neutrophils by binding to the G-CSF receptor on hematopoietic cells. This combination is specifically designed for hematopoietic stem cell mobilization prior to autologous transplantation in patients who have difficulty achieving adequate CD34+ stem cell counts with standard regimens. The addition of pegfilgrastim reduces infection risk by promoting neutrophil recovery after chemotherapy-induced myelosuppression[1][2][3][5].
