The regimen consisting of etoposide, nab-paclitaxel, irinotecan, and lurbinectedin refers to a selection of chemotherapy options being evaluated by Sichuan University in a Phase 2 clinical trial (NCT05686317). This study investigates the safety and efficacy of combining liver-directed radiotherapy (low-dose radiation or stereotactic body radiation therapy) with immunotherapy (PD-1/PD-L1 inhibitors) and a choice of one of these four chemotherapy agents. The treatment is intended for patients with pre-treated extensive-stage small-cell lung cancer (ES-SCLC) or non-small cell lung cancer (NSCLC) who have developed liver metastases and have failed prior therapies. Etoposide and irinotecan are topoisomerase inhibitors, nab-paclitaxel is a nanoparticle albumin-bound microtubule stabilizer, and lurbinectedin is a selective inhibitor of oncogenic transcription that binds to the DNA minor groove and inhibits RNA polymerase II. The trial aims to determine if this multi-modal approach can improve outcomes in a population with a poor prognosis.
