**EV-NHC** is an investigational, injectable extracellular-vesicle delivery formulation developed at Johns Hopkins University for **Crohn's disease perianal fistula**. It consists of mesenchymal stem cell-derived extracellular vesicles retained within a biodegradable nanofiber-hydrogel composite microgel matrix. The hydrogel carrier is designed to prolong local EV retention and enable sustained release at the fistula site, supporting anti-inflammatory and regenerative activity. In a rat model of Crohn's disease perianal fistula, EV-NHC reduced local inflammation and improved fistula healing, with findings consistent with macrophage polarization and enhanced neovascularization. It remains a preclinical research-stage therapeutic platform.
