Evacetrapib is a potent and selective small molecule inhibitor of cholesteryl ester transfer protein (CETP), developed by Eli Lilly. It was designed to elevate high-density lipoprotein (HDL) cholesterol and lower low-density lipoprotein (LDL) cholesterol by inhibiting CETP-mediated lipid exchange between lipoproteins. Unlike earlier CETP inhibitors such as torcetrapib, evacetrapib does not increase aldosterone or blood pressure. The drug advanced to phase 3 clinical trials for the treatment of dyslipidemia, hyperlipidemia, hypercholesterolemia, and high-risk cardiovascular disease but was discontinued due to insufficient efficacy in reducing cardiovascular events despite favorable changes in lipid profiles[1][2][3][5][7]. Preclinical studies also suggest potential for repurposing in Alzheimer’s disease due to its ability to enter brain tissue and modulate cholesterol metabolism[6].
