Evategrel (CG-0255) is an oral small molecule P2Y12 receptor antagonist antiplatelet agent being developed by CureGene Pharmaceutical. It is a thioether prodrug that undergoes simple hydrolysis by carboxylesterases to form the active metabolite H4, which is the same active metabolite produced by clopidogrel. By utilizing this metabolic pathway, evategrel bypasses the CYP2C19 hepatic enzyme system, thereby overcoming the genetic variability and resistance often associated with clopidogrel treatment. The drug is designed for rapid onset of action, achieving significant platelet inhibition within 15-30 minutes. Evategrel is currently in Phase 2 clinical development for the treatment of cardiovascular and cerebrovascular conditions, including acute coronary syndrome (ACS), recent myocardial infarction (MI), recent stroke, and established peripheral arterial disease (PAD). It is notable for being the first P2Y12 inhibitor available in both oral and intravenous formulations, facilitating a seamless transition from emergency to maintenance therapy.
