Overview

This is a combination therapy consisting of three small molecule drugs—everolimus, exemestane, and metformin—investigated primarily for the treatment of metastatic, hormone receptor-positive, HER2-negative breast cancer in overweight and obese postmenopausal women. Everolimus is an mTOR inhibitor that blocks the mammalian target of rapamycin pathway involved in cell growth and proliferation. Exemestane is a steroidal aromatase inhibitor that suppresses estrogen production by irreversibly binding to the aromatase enzyme. Metformin is an oral antihyperglycemic agent commonly used for type 2 diabetes; it also has antiproliferative effects via activation of AMP-activated protein kinase (AMPK) and inhibition of mTOR signaling. The rationale for this combination targets multiple pathways implicated in breast cancer progression, especially those exacerbated by obesity such as increased estrogen production and insulin resistance. In phase II clinical trials, this regimen demonstrated moderate clinical benefit with manageable safety in the specified patient population[1][2][3].
