Overview

The combination of everolimus, letrozole, and trastuzumab is a triple therapy approach that has been studied in clinical trials for hormone receptor-positive, HER2-positive advanced cancers, particularly breast cancer. This combination targets multiple pathways simultaneously to overcome resistance mechanisms. ## Mechanism of Action This triple combination works through complementary mechanisms: - **Everolimus** is an mTOR inhibitor that blocks the PI3K/AKT/mTOR pathway, which is often activated in resistant breast cancers - **Letrozole** is an aromatase inhibitor that reduces estrogen production, targeting hormone-dependent tumor growth - **Trastuzumab** is a monoclonal antibody that targets the HER2 protein, inhibiting HER2-mediated signaling The rationale behind this combination is that dual inhibition of estrogen receptor (ER) and HER2, along with mTOR inhibition, may provide synergistic anticancer activity in tumors that express both hormone receptors and HER2. ## Clinical Development A phase I dose-escalation study (NCT02152943) evaluated this combination in patients with hormone receptor-positive, HER2-positive advanced cancers. The study determined that the recommended phase 2 dose (RP2D) was: - Trastuzumab: 8 mg/kg loading dose followed by 6 mg/kg maintenance dose IV on day 1 - Everolimus: 10 mg daily orally - Letrozole: 2.5 mg daily orally - Treatment cycle: 21 days The most common adverse events included mucositis, hyperglycemia, fatigue, and hypertriglyceridemia. Dose-limiting toxicities included grade 3 mucositis and grade 4 neutropenia. The combination showed promising clinical activity in heavily pretreated patients, with partial responses observed in several breast cancer patients with HER2 amplification or mutation.
